Patient Participation Group.

The next Patient Participation Group meeting is to be announced. These meetings are open to all registered patients so please come along if you are free. Your ideas and views are important to us and can help us shape services to our patient's needs.

 Appointments not attended

In the month of February a total of  82 patients did not attend their booked appointments ( 8 with the doctors and 76 with the practice nurse) nor were the appointments cancelled so they could not be offered to patients in need of them. If you no longer need or are unable to attend the appointment you have booked, please call the surgery on 020 7266 1449 to cancel it.

 

New Patient Registration

consultation1The doctors welcome new patients who live within our practice area. 

Woodfield Road Surgery is open to eligible NHS patients who wish to register with us. All information given to the practice is treated as confidential and would only be disclosed within the legal requirements of the data protection act. The Practice policy is to accept any eligible person and not to discriminate against anyone, regardless of any pre-existing health conditions, required treatment, age, gender, race, ethnicity, religion, disability, sexual orientation or trans status.

No proof of identity is required.

All staff at Woodfield Road Surgery receive annual Equality and Diversity training.

People who live in W9 and parts of W2 may register with the surgery by visiting us during the surgery opening hours. If you are already registered and intend moving out of the practice area, you must inform us so that arrangements can be made for you to register with another doctor in your new area. Please telephone the surgery on (020) 7266 1449 if you require further information.

If you are registering children under the age of 16 years then please bring their Red Book (Child Health Surveillance Book) when you come in for your registration appointment.

You may print off a registration form and a new patient questionnaire, fill it out and bring it in with you on your first visit to the practice.
