New Patient Registration
The doctors welcome new patients who live within our practice area.
Woodfield Road Surgery is open to eligible NHS patients who wish to register with us. All information given to the practice is treated as confidential and would only be disclosed within the legal requirements of the data protection act. The Practice policy is to accept any eligible person and not to discriminate against anyone, regardless of any pre-existing health conditions, required treatment, age, gender,race, ethnicity, religion, disability, sexual orientation or trans status.
All staff at Woodfield Road Surgery receive annual Equality and Diversity training.
People who live in W9 and parts of W2 and W10 may register with the surgery by visiting us during the surgery opening hours. If you are already registered and intend moving out of the practice area, you must inform us so that arrangements can be made for you to register with another doctor in your new area. Please telephone the surgery on (020) 7266 1449 if you require further information.
If you are registering children under the age of 16 years then please bring their Red Book (Child Health Surveillance Book) when you come in for your registration appointment.
You may print off a registration form and a new patient questionnaire, fill it out and bring it in with you on your first visit to the practice.
